We are asking if you need to put in a message to the nurse, need a copy of your child's vaccination record, need to see your child's lab results, or any other medical records that you do so through our patient portal. All of these items are available and free on your child's portal. Beginning August 1, 2025, there will be a fee of $5.00 for our office staff to print any medical record form that is available on your child's patient portal.
